Groom Waiver

All new clients are required to fill out this document as it will allow us to have all your contact information while your pet(s) is / are in our care. In addition to knowing more information about your pet. This document is only required once and is kept on file. If you have someone other than yourself dropping off your pets on your behalf please be sure to have these documents filled out for them to provide us so we have YOUR information as he legal pet parent. In addition if you have another party dropping off on your behalf, please send them with grooming instructions / what kind of haircut (if applicable) you are requesting. If anyone other than yourself is picking up your pet(s) please provide us with their name and number.

For pets over the age of 10 years

This waiver is only needed for pets close to or over 10 years of age. This waiver is to ensure pet parents are aware of the precautions we take when grooming a senior pet.

Vaccine Proof Required

By choosing our salon or mobile vehicle to pamper your pet,  you are agreeing to providing one of these forms of vaccine document’s. We do all we can to ensure clients are aware of this policy, from our web content to discussing the requirements when making an appointment over the phone.

Email’s are sent automatically when an appointment is set and two days prior to a scheduled appointment. We also call the day before the scheduled appointment to remind the pet parent(s) of what is needed. We are diligent with this process and try to be as helpful as possible.

We do all we can to ensure our appointment’s are maintained, as we don’t want appointment discrepancies due to missing vaccines, thus we try to set up potential clients up for success. However at times, these efforts are over looked. We are not to be held responsible for a clients over sight in our efforts in regards to our vaccine policy.

Vaccine Policy

Due to the high number of last minute cancellations, no shows, refusal of services due to lack of documentation by new clients (if a client chooses to bring in vaccine documents.) [Will not apply for new clients whom have had valid documentation provided prior to appointment.] New clients will be limited to scheduling their first appointment with us to a appropriate time slot.

Any late notice cancellations, no shows and refusal of services limit our abilities to fill these time slots in a appropriate  manner. We apologize for this inconvenience. To book your desired time there are many ways we can obtain records prior to your appointment.
